One of the issues with the Apollo I fire was that the hatch did not have explosive bolts and, as depicted, required a wrench and a multi-step procedure to close and open the hatch. This, ironically, was due to a defective explosive hatch on Gus Grissom's Mercury flight that blew after splashdown, causing the loss of the capsule. If the hatch had not accidentally blown on Grissom's spacecraft, his Apollo I capsule would probably have had the explosive bolts for fast egress and it might have saved the life of at least one of the astronauts and perhaps even Grissom.
