The "astral battle" in the hospital draws heavily on a Doctor Strange graphic novel called The Oath. This sequence is the one he wrote in advance, essentially on spec, to get the job directing the movie. It was a twelve-page scene that was part of his ninety-minute presentation to Marvel.
