There are many hints and clues in the movie that foreshadow the plot twist that reveal that Esther is really an adult: When Kate tries to talk to Esther about the birds and the bees after witnessing she and John having sex, Esther says that she is aware that adults have sex, showing that she has an unusually large amount of knowledge about sexual relations for someone who's supposedly 9. Esther displays a high amount of intelligence, even for a supposed child prodigy, being a skilled pianist, a talented singer, painter, and being able to learn new languages very quickly. When the school bully, Brenda, at Daniel and Esther's school makes fun of Esther for the clothes she wears and for carrying a bible, she sheds no tears. Esther claims to never have played a piano, yet later she plays the piano perfectly implying that she has been playing the piano for a long time. Esther takes the longest to get ready for her first day of school, because she needed to apply all of her age defying makeup that makes her look like a child. Esther insists on always wearing dresses instead of jeans, because she's trying to hide her adult figure. Esther never removes her ribbons or collars because she's hiding scars from when she tried to escape her straitjacket. Esther always cuts up her meals into tiny pieces before eating, because she is wearing fake teeth to hide her actual rotted teeth, the former of which would make it difficult to chew up her food. Esther continuously avoiding the dentist and clinic appointments because she doesn't want her secret, that she's actually 33 years old, to be discovered. This unfortunately creates a plot hole with the scene where she purposely breaks her arm to frame Kate, as she's wearing a cast later, showing that she went to the hospital for it. The doctors would have easily discovered that she's actually an adult with a simple bone scan showing that she has the bone structure of an adult, and not a still developing child. Esther is pretending to be Russian, because she's actually a 33 year old Estonian, which would mean she was born in what was still the Soviet Union. It's very likely that she attended a Russian school and became fluent in the language. Estonia is a small country, including in population, and Esther claiming she's from Russia gives her a lot of ground to cover in explaining her heritage to other people. Esther being fluent in Russian wouldn't be suspicious because that could simply be her native language.
