There are several mentions about the Il Mostro case in this movie and it is revealed that the Italian Cop that was investigating the murder was moved off the case to investigate the disappearance of 'Doctor Fell's' predecessor at the Caponi Library in Florence, Italy. During the introduction scene where Hannibal and the Inspector meet, the camera focuses oddly on a janitor buffing the floors. Also, in the scene at the Library where the Inspector is captured by Hannibal and hanged, the camera does the same thing on the janitor. In the deleted scenes of the movie, it is revealed that the janitor is Il Mostro and that Hannibal and him were correspondents, much like Hannibal was with Francis Dolarhyde. This further shows how much a poor cop the Inspector was, as he had two serial killers right under his nose and he didn't know he had at least one under his nose until it was too late.
