At the beginning of the movie, beginning at 3:55, the Dr is cutting Bourne's microchip out. No Dr would ever handle, or cut with a scalpel in that way, using the tip, instead of sliding it at an angle, utilizing the actual blade. The Dr instead, uses the tip only & jaggedly forces the incision. Handling a scalpel properly would be intrinsic for any Dr, especially one at his age, regardless of alcoholism.
