Michael kept remarking that Rosaria needed to see a shrink as opposed to a priest. In real exorcisms, the rite cannot proceed until all reasonable doubt is removed via investigating medical issues before moving on to mental then spiritual issues (family history of mental health, any medications/drugs used, etc.). This probably was to further create doubt in Michael's faith in God, but it backfired when Michael confronted Ba'al (through Rosaria in the hospital) that if Ba'al didn't exist, as he claims, then Rosaria isn't really possessed, which frustrated Ba'al.
