The accident of the transporter kills Lt. Commander Sonak and an unnamed female crew member. In the film's novelization and the Blu-ray audio commentary for the Director's Edition, it is revealed that the female crew member is Vice Admiral Lori Ciana, Admiral Kirk's love interest who unknown to him was transferring to the Enterprise as navigator. Her death causes the last-minute replacement by Lieutenant Ilia. This explains the surprise of Commander Decker at Lieutenant Ilia's arrival when announced by Uhura.
