According to the audio commentary by Paul W.S. Anderson, he mentions that the character of Dr. Isaacs (Iain Glen) is an homage to actor Jason Isaacs, who made an uncredited cameo as Dr. Birkin and the film's narrator during the opening credits of the first film. Dr. Birkin as a character was meant to be someone who's backstory would be expanded in further films but was scrapped when Jason Isaacs left this project. The character Dr. Isaacs was created as a replacement.
