It's not revealed at the end of the movie if it was a dream or it was real. It is was a dream, it means Quaid has chosen the dream over reality and that Dr. Edgemar was telling the truth and as of consequence, Quaid is lobotomized. If it was real, Dr. Edgemar would most likely had been lying about Quaid's dangerous adventure on Mars being a dream and does it to manipulate Quaid in to taking the red pill which knock Quaid out and so Richter can drag him back to Cohaagen and have his former personality Hauser and his memories as Hauser restored.
