When running away from the monster and the magma into the tunnels on Monster Island, General Horne and Sarah Murray end up at a gate which is locked with a padlock, leaving them trapped. When the magma blocks the other way out, they go back to the gate and General Horne kicks it open. The padlock is clearly missing and the gate is only 'secured' with a chain very loosely wrapped around the gate.
