(at around 1h 15 mins) It's nearly inconceivable that the children would have been sent to their grandparents' home without first speaking with them on the phone. Not being able to recognize their voices should have been the first clue as to their identity not being correct.
However, the mental patient pretending to be the grandfather mentions that the real grandparents already knew about the children coming to visit them, therefore it's clear that the Mom set up the meeting when they were still alive.
