In the final scene, Brian is killed in an explosion because Rory saved him from getting hit by a news van thus altering Death's plan. But the news van was only there because of the car crash survivors and his death was never part of Death's plan. Had the initial premonition never occurred the group would have died, the news would have had no reason to go out to the farm, and Brian would not have needed saving at all.
