This film heavily implies that the airborne anti-virus that Isaacs had planned to release to cleanse the world of the T-Virus in order to repopulate was not a new invention. This brings into question on why the Red Queen never mentioned its existence in the previous films, or any other Umbrella executive for that matter. Furthermore, why did Umbrella only develop one vial if they were planning to cleanse the whole world with it? As Alice explains in the ending, the anti-virus is spread by wind, so it could take years for it to reach every corner of the earth.
