Laura is going to use the hook and pliers to remove the bullet from Levi's shoulder, except when dropping them in the boiling water to disinfect them, she doesn't open the pliers. The inside of the pliers would need to be disinfected too, which is common sense and as a surgeon (or anyone w/ common sense about this sort of thing), she would know that.
