Vincent tells Heather that it's only possible for a Silent Hill cultist to escape by going through a torturous trial that includes having the Order's symbol carved into your chest. Despite this, it's inferred that Claudia can actually leave Silent Hill anyway, as the shopping mall sequence shows her monster form, the Missionary, attacking and killing Douglas Cartland. There is no evidence that she herself also went through a trial that involved carving a symbol into her chest, or Douglas for that matter, since it's not known if he's actually from Silent Hill or not.
