When the monster went into the basement after Evelyn, not only would a being with super-sensitive hearing hear her panting, it would also hear the two heartbeats, one of the mother and one of the unborn child. However, the ticking of the timer clock clearly acts as "white noise" in the room which would mask any smaller sounds such as heartbeats. When the creature's head shell opens revealing its "ear", we hear it focusing on the timer clock.
