Ethan and Sarah doesn't share same height and maybe even the physique. They should have as they are same person. However, based on the surgeon's statements near the beginning of the film, this could be explained by being part of the reconstruction and grafts from exposure to the explosion (or possibly a second, off-screen incident, as the Blackmore Cross medal, presumably for injury in the line of duty, was the second one that John received).
