It is never revealed how Monday found out she was pregnant with twins. Presumably, given the one child rule in that society, any doctor she had consulted would have been required to report it to the authorities. Also, she is not far enough along to be showing, and yet the babies rescued from her dead womb are more advanced than her size would have indicated.
