The narrator states that Adaline's heart had stopped and that the paramedics waited the prescribed 5 seconds before delivering the shock to Adeline. Unlike a hospital setting with a traditional defibrillator, the paramedic is using an AED. These devices tell the responder when to administer a shock, and it will only do so if the machine sees certain cardiac rhythms. Asystole, or a total lack of a heartbeat, isn't one of them. No shock would be delivered in this situation, and there is no override that one could use to do so.
