As a Type 1 diabetic, even without food/calorie intake, diabetic ketoacidosis (DKA) can develop in a matter of hours without insulin. Hyperglycaemia by itself may not be fatal if DKA does not develop. DKA needs to be treated not only with insulin, but with massive amounts of fluids and potassium in a hospital setting. Subcutaneous insulin may not stop the process and it can lead to death within hours to days.
