(at around 1h 9 mins) When Marcus & Blair punch their way out of the vent shaft, the cover lands on a mine. The blast propels it up and it lands on top of the vent outlet. The cover that lands there is obviously attached to a string or cable that keeps it from falling off the edge. Additionally, the vent cover that landed on the mine is still visible in that position before the camera moves in on Marcus and Blair.
