Microbes were unknown at the time of the Civil War and therefore doctors did not sterilize instruments or wounds.He wasn't sterilizing the wound. He was also unconcerned about sterilizing the probe he got the bullet out with. Could be he was merely torturing Marlowe a little bit. Payback for poor attitude toward doctors.
